CAUSE OF DEATH: accidental drowning via "shallow water blackout." It occurs when a swimmer pushes themselves to their limit, but continues on without stopping. The overexertion and lack of oxygen causes their body to black out on the spot. Doing so can be fatal. In Haruka's case, he'll have been pushing himself during his training, wanting to keep going no matter what, just to improve himself. Eventually, his reckless desire to keep going will result in him suffering "shallow water blackout," and he'd more than likely die on the spot.
